PHI is encrypted in transit and at rest, access is strictly controlled, data is backed up regularly, and every action is logged. How is DashQuill Forms different from other online forms?
With most online forms, patients fill them out and practices receive a PDF that must be manually typed into the patient's chart. With DashQuill Forms, patient responses automatically populate the chart. Staff can review what's changing and click 'Import'—no typing or copying anything.
What types of forms are supported?
Any patient form. Upload what you already use, such as registration forms, medical history, consent forms, HIPAA forms, and specialty-specific intake. Our AI converts each one into a digital form. Some offices start with just their new-patient registration form, and others convert their entire paperwork packet.
How can patients access forms?
Wherever is easiest for them. Forms can be texted or emailed to a patient directly from the EHR, included in their appointment reminder, embedded on the practice website for new patients, or completed on a kiosk or tablet in the waiting room. No matter where patients fill them out, everything flows into one forms inbox in the EHR.
Can patients upload files?
Yes. File uploads can be added to forms. Patients can upload insurance cards, ID photos, medical records, or any other documents. When the form is imported, those files are automatically uploaded to the patient's chart. There is no need to download them to a computer, upload them to the EHR, and then clean PHI off the computer.
How long does onboarding take?
Most practices are up and running within a few days. Our AI converts your existing forms in minutes, and the EHR connection is set up during onboarding. The main variable is how many forms you're bringing over and how much you want to review before going live.
