AdvancedMD Patient Forms

Patients complete intake on their phone. Your front desk reviews what is about to change and clicks Import. The answers land in AdvancedMD, on both the billing side and the clinical chart. Nobody types intake in twice.

Trusted by AdvancedMD practices nationwide

One submission, both records

The double entry between the PM and clinical sides is the specific problem this solves for AdvancedMD practices.

1

Send from the chart

Text or email the form, attach it to a reminder, publish it on your website, or use a tablet at the desk. Patients fill it out in a browser.

2

Answers map to both sides

One submission routes demographics and insurance to the practice management record and clinical answers and documents to the chart.

3

Import to AdvancedMD

Staff check the proposed changes and click Import. Uploads file with the right AdvancedMD document type already assigned.

What practices say

Writing to the PM and the EHR from one import

This is the capability that separates AdvancedMD from our other integrations. Insurance and demographics captured at intake reach the practice management side, where they matter for claims, without anyone rekeying them from the clinical chart.

  • Write to both the PM and EHR sides of AdvancedMD
  • Insurance details captured once and applied where billing needs them
  • Demographics and contact information auto-fill
  • Emergency contacts added to the chart

Pharmacy and attachments land where they belong

The preferred pharmacy writes into AdvancedMD directly. Every uploaded file is assigned a document type, so it can be found later instead of sitting in an unsorted pile.

  • Set the preferred pharmacy on the AdvancedMD chart
  • Correct AdvancedMD document type assigned to each upload
  • Signed forms, insurance cards and photo ID file to the chart
  • Supporting documents from the patient come across too

Your paperwork, minus the clipboard

Practices do not want their forms redesigned. They want them to stop being paper. Existing forms are converted with the wording intact and gain the things paper cannot do.

  • Existing forms converted during onboarding, wording and logo kept
  • Conditional questions so patients skip what does not apply
  • E-signatures and file uploads built in
  • Scored assessments such as PHQ-9 calculated automatically

Questions we get asked first

The things AdvancedMD practices want settled before a demo.

Does one submission really update both the PM and the EHR?

Yes. Among our integrations, this is specific to AdvancedMD. Demographics and insurance reach the practice management side, while clinical answers and documents go to the chart. It all comes from a single import rather than two rounds of entry.

Can a form create a new AdvancedMD patient?

No. The patient has to exist in AdvancedMD before a submission can be imported. Creating a patient from a form is available on Practice Fusion only.

What happens to insurance cards?

The photo uploads to the AdvancedMD chart with the correct document type. The insurance details the patient typed are written as structured fields separately, so you get both the data and the image.

How is this different from an online form we email out?

An emailed form gives you a document to read and retype. This writes into AdvancedMD fields on both sides, so what is left is reviewing the proposed change and clicking Import.

How long does setup take?

Most practices go live within a few days. Existing forms are converted and mapped to AdvancedMD during onboarding. The timeline depends mainly on how many forms you bring across.

Is it HIPAA compliant?

Yes. PHI is encrypted in transit and at rest, access is controlled and logged, and a Business Associate Agreement is signed during onboarding. Our HIPAA compliance page has the detail.

The demo takes fifteen minutes. It is worth it if your staff currently enters intake twice, once per side.
